Fullness under the chin seen in profile

Double Chin

Weight is only one of four reasons for a double chin, which is why so many people lose weight and still have one, and why the right assessment changes everything. At our Berkhamsted clinic we work out whether yours is fat, skin, muscle or chin position, then match it to CoolSculpting Elite, Exion radiofrequency, chin filler or a jawline-sharpening injectable plan.

What it is and why it happens

The most common assumption about a double chin is that it is weight, and the most common frustration is losing the weight and still having one. Four things create fullness beneath the chin, and they turn up in different combinations. Submental fat is a discrete pad that is stubbornly indifferent to diet and exercise, often genetic and present at a perfectly normal body weight. Skin laxity produces the same silhouette without any excess fat, particularly after weight loss. A weak or separated platysma, the sheet of muscle across the front of the neck, loses the definition that once held the jaw edge. And a recessed chin or small jaw creates the look of fullness without fat or laxity being involved at all.

Pinch beneath your chin. If you can grasp a distinct pad, there is fat. If the skin comes away thin and loose with little substance, it is laxity. Matching the treatment to the cause is what makes the result land, and at our Berkhamsted clinic that assessment comes first, with CoolSculpting Elite, Exion radiofrequency, chin filler and jawline injectables all under one roof in Berkhamsted.

Which type is yours?

A distinct pad you can pinch
Submental fat

Genetic in many cases and present at a normal weight, which is why diet so often fails to shift it. CoolSculpting Elite reduces the pad directly, with no needles and minimal downtime. The most straightforward type to treat and the one with the clearest result.

Loose, thin, little to pinch
Skin laxity

Common after weight loss and with age. The silhouette is the same but there is no fat to remove, so the answer is tightening rather than reduction, and Exion radiofrequency on the neck and jawline is built for exactly this.

Vertical neck bands, lost jaw definition
Platysma laxity

The muscle sheet across the neck has weakened or separated, so the jawline loses its edge and vertical cords appear. Anti-wrinkle injections along the jaw and upper neck release the downward pull and sharpen the edge.

Weak chin projection in profile
Recessed chin

Neither fat nor laxity. A small or set-back chin shortens the distance between chin and neck, creating fullness in profile. Chin filler projects it forward and can noticeably improve the profile with no fat touched at all.

How we assess and treat

Assessment at our Berkhamsted clinic is done sitting upright and in profile as well as straight on, because a double chin is largely a profile problem and the profile is what you see in photographs. The pinch test does most of the diagnostic work: a distinct pad between the fingers means fat, thin loose skin with little substance means laxity, and both together is the most common finding over forty.

We check chin projection in profile, because a recessed chin is regularly the main driver and regularly missed, and plenty of people who have had fat reduction elsewhere with a quiet result turn out to have a chin position that nobody assessed. We ask about weight trajectory too, and anyone actively losing weight is usually advised to let things settle first, since further loss changes both the fat and the skin. Then the plan: CoolSculpting Elite for a defined pad, Exion radiofrequency to tighten the neck and jawline, chin filler for projection, and anti-wrinkle injections along the jaw where the platysma is pulling the edge down. Your clinician covers suitability, timing and aftercare at your consultation, and the team is a phone call away afterwards.

Can you treat it at home?

Weight loss reduces submental fat where the fat is weight related, and does nothing where the pad is genetic. Plenty of slim people have one, which is the clearest evidence that it was never simply about weight. Posture makes a real difference to how it looks, if not to the anatomy: hours spent looking down at a phone shorten the front of the neck and accentuate fullness, and it is worth noticing how much of your day is spent that way.

Neck exercises and jaw-toning devices do not remove fat or tighten skin, and some of them work the platysma in a way that makes the lower face look heavier rather than lighter. Skincare on the neck helps texture rather than contour, though vitamin A skincare and SPF applied to the neck as faithfully as the face slow the thinning that feeds laxity over the years. Nothing at home reaches a fat pad or a lax platysma, which is why this is one of the concerns where clinic treatment makes the most visible difference.

Frequently Asked Questions

Four things: submental fat that is frequently genetic, skin laxity, a weak or separated platysma muscle, and a recessed chin. They occur in combination, and working out which dominates is what decides the treatment.

It helps where the fat is weight related and does nothing where the pad is genetic. Plenty of slim people have a double chin, which is the clearest sign it was never simply about weight.

Pinch beneath your chin. A distinct pad between your fingers is fat. Thin loose skin with little substance is laxity. Over forty it is commonly both, and your clinician confirms it in profile at your consultation.

For a defined fat pad, CoolSculpting Elite. For laxity, Exion radiofrequency on the neck and jawline. For a recessed chin, chin filler. For lost jaw definition from the platysma, anti-wrinkle injections along the jaw. Most people over forty combine two.

CoolSculpting Elite for the double chin is £900 for two cycles, Exion needle-free radiofrequency from £180, chin filler £400 and a Nefertiti lift £350 at our Berkhamsted clinic.

Yes, on a defined pad you can pinch. Controlled cooling destroys the fat cells, the body clears them over the following months, and you see the change from one to three months, still improving at six. No needles, minimal downtime, and the chin usually takes two cycles.

Two cycles for CoolSculpting on the chin, six weekly sessions of Exion then a top-up every two to three months, one for chin filler and one for a jawline injectable plan. Most plans combine two approaches.

Not meaningfully. Neck and jaw exercises do not remove fat or tighten skin, and some jaw-toning devices build muscle in a way that makes the lower face look heavier.

Where the issue is laxity rather than fat, yes. Needle-free radiofrequency tightens the skin of the neck and jawline with nothing more than a gentle warmth, and it pairs beautifully with CoolSculpting where there is fat as well.

Where the chin is recessed, frequently dramatically. A set-back chin shortens the chin-to-neck distance and creates fullness in profile with no excess fat present. It is the most commonly missed cause, and the most satisfying to correct.

If you are actively losing, yes, let things settle. Further loss changes both the fat and the skin, and treating mid-journey is treating a moving target.

Yes, submental liposuction or a neck lift, and for significant fullness with a lot of excess skin that remains the most complete answer. Your clinician will tell you at your consultation if that is where your best result lies.
